D1 forms are required to validate security information requests from manufacturers, by verifying identification and ownership authorization. A Contracting D1 is required when a Vehicle Security Professional (VSP) is performing work on a customer owned vehicle for another shop.
To begin a Contracting D1 Form:
1. Log in to the SDRM portal (https://sdrm.nastfsecurityregistry.org)
2. Select the D1 Forms link from the menu bar
3. Select the Customer D1 Form
4. Gather required Information:
- Vehicle: VIN, year, make, model, color, mileage (if available)
- License plate (country/state, if available)
- Purchase Order or Request Number (optional)
- Owner’s valid U.S. or Canadian driver’s license number
Note: Documents such as state issued ID cards, passports, foreign identification and immigration forms are NOT accepted for ownership authorization
5. Choose a Verification Type:
- (e.g., registration, insurance, bill of sale, or title certificate)
6. Verify Ownership in person
7. upload ownership documents
8. Complete the Service Info:
- Describe the service performed and add notes under “other” (if needed)
9. Submit the D1 Form (this automatically links to the OEM transaction)
Important:
- The D1 form can be saved at any time to complete later (form appears on your dashboard)
- NASTF policy requires vehicle information and owner verification is acquired prior to performing any security operation
- Every security-related OEM transaction requires a completed D1 form
- Unresolved OEM transactions over 5 days will lock your account
